What is CONNECT?

CONNECT strengthens the communities we serve, through engagement in community settings. CONNECT enhances traditional outpatient services by meeting adult clients’ individualized needs at their own location.

CONNECT works with communities they serve to collaboratively identify specific community needs and creates programs targeted to the community. Our highly skilled and experienced team delivers care to those in need. CONNECT is a joint project with NYC DOHMH that strengthens and expands the integration of clinics into communities.

CONNECT is a groundbreaking initiative that aims to foster continuous engagement between community and clinic treatment services. It goes beyond the confines of traditional outpatient care by reaching clients in their own environments and addressing their individualized needs directly at their preferred locations.

The core principle of CONNECT is to collaborate with the communities we serve, working closely with them to identify specific needs and develop tailored programs that address those needs effectively. By actively involving the community in the planning and implementation process, we ensure that our services are aligned with the unique challenges and requirements of each community.

This partnership allows us to leverage collective expertise and resources to extend the reach of our services and expand the integration of clinics into communities. By combining our knowledge, skills, and networks, we can effectively address the diverse needs of individuals and communities, promoting positive health outcomes and fostering lasting connections.

Outpatient mental health and substance use services

A CCBHC offers expanded coordination with other health care and social service providers, with a focus on whole health and comprehensive access to a full range of medical, behavioral and supportive services.

What will I receive:

CONNECT offers an expanded variety of outpatient mental health services. CONNECT offers coordination with a host of community services to ensure that you receive holistic services. CONNECT offers comprehensive access to case coordination, health care support, and other community needs.

CONNECT Convenience:

CONNECT services are conveniently provided in your home, community, or clinic. Services are provided in your language. Walk-In clients are always welcome. We accept all eligible referrals regardless of insurance or immigration status.
