Children’s Care Management Services
This program provides supportive care management services to mentally, behaviorally, or emotionally challenged youth throughout Brooklyn. Through home visits, families are provided with ongoing assistance and advocacy to access mental health, education and income support programs that are essential to meeting their child’s needs.
